Financial Aid Forms

Tired of waiting in line? Save yourself time by printing up your forms online.

Before you get started here are Five Good Tips you may need before completing our forms.
Tip 1: Be sure to use the correct year's forms when completing the FAFSA Application.
  • 2016-2017 forms for Fall 2016, Spring 2017 and Summer 2017
Tip 2: Complete and send verification and tax forms only if requested to do so. If selected for verification you will receive a student email with a link to the required worksheet and instructions.
You will have to be signed into MyCampus Portal to access the linked forms.
Tip 3: Verification forms must be fully completed and signed; please be sure to check that no items have been left blank before sending the forms to your campus Financial Aid Office.
Tip 4: If you use estimated income information on the FAFSA, be sure to make a correction to the information as soon as your actual income information is available. If you are selected for verification the Financial Aid office will make these corrections for you.
Tip 5: If you are a new applicant for the CAL Grant, be sure to complete your FAFSA and submit your CAL Grant GPA Verification form in advance of the March 2nd deadline.  The California Student Aid Commission does not make exceptions to the March 2nd deadline.
