Procedure to Petition for a Certificate
Certificate Petition Form
- AFTER completing ALL requirements for the certificate, student must obtain a “Certificate Petition” card located in the lobby area across from the Admissions and Records Office or download and print out the Certificate Petition Form
- Student completes the petition card and returns it to the Admissions and Records Office.
- The Admissions and Records Office sends the petition card and a copy of the student’s transcript to the proper Executive/ Associate Dean.
- Dean verifies eligibility, grants/denies certificate, and mails granted certificate to student.
- Dean authorizes petition card as granted and returns it to the Admissions and Records Office.
- Once “authorized” petition card is received, Admissions and Records Office will post to transcripts.
